I heard the chaos this morning. Wicomico 911 dispatched an ambulance from Station 5, Hebron Fire Department to a house Old Ocean City Road in Station 2's district for a non breathing subject. They then dispatched a fire engine 16-1 from station 16 for a medical assist. The fire engine crew then told the ambulance from Hebron to go to another call and asked for a Fruitland Fire Department ambulance from the hospital. They worked to poor person for a while and then called for the police and the ME. No one has a chance when you have to wait for an ambulance to come from Hebron when you live closer to Parsonsburg. The non breathing patient needed an ambulance not a fire truck. Someone needs to look into this.

Actually that fire engine/truck has an ALS seal of excellence from MIEMSS. This means that it has nearly all the same equipment on it that an ALS ambulance does. Oh they also have a FF/paramedic that always staffs the truck. The reason the rerouted Hebron was because it was only staffed with EMTs, that's why the marked up as A5 and not paramedic A5. Fruitland was on their way to another call in sta 1's district which was of a lower severity. So they rerouted the BLS A5 to that call and called for paramedic B3 (Fruitland) to respond to the nonbreathing call.

No for a little education: research shows that less than 10% of all "nonbreathing" cardiac arrest patients actually survive to walk out of the hospital. Due to this MIEMSS Maryland EMS protocols have been updated with TOR (termination of resuscitation) procedures. This means that if a paramedic is called to a scene for a nonbreathing subject they are to perform their resuscitation efforts on scene. Research shows that better resuscitation efforts are performed when the patient is left in place and not moved to the ambulance. If after 15 minutes of the proper resuscitation efforts are performed and there has been no change in the patients condition (ROSC- return of spontaneous circulation) then the paramedic is to contact medical control via radio and confirm death with online physician. The paramedic is to then contact central and request PD and an ME.

No I don't work for SFD, but I am a paramedic and I obviously know a lot more about the subject then you do.
