The Long Island Rail Road should have foreseen the potential for a morning rush-hour crash that was caused by an engineer's undiagnosed sleep apnea and injured more than 100 passengers at Atlantic Terminal in 2017, according to a new federal report.
The National Transportation Safety Board criticized the LIRR for not doing enough to manage the risks of "end of terminal accidents" -- despite having been involved in similar incidents in recent years. LIRR officials said there were four on the railroad since 2007. In only one of them before the Brooklyn crash at Atlantic Terminal were there injuries, LIRR officials said.

anything to cover their asses, I remember when I was young and was in the county lockup in a western Maryland county and 5 guys in my cellblock escaped by busting out the ceiling and making their way to freedom.The guards didn`t discover it until 12 hours later even though they did 2 cellblock checks during that time. They covered their asses by claiming they had bad eyesight and didn`t see the massive hole in ceiling. a couple days later they were all wearing glasses LMAO. the truth is they were lazy doing their cellblock inspection and never even went into the shower stall to check it.The escapees busted the ceiling out of the shower stall and made their way across the ceiling to the front of the jail where the administrative officers were and pushed out an unsecured air vent on the side of the building that lead to the outside. If my memory serves me correctly I believe that was in 1987 in carroll county, Maryland. the guards covered their asses,kept their jobs and got a free pair of eyeglasses out of the deal.
